I need a template document for a health and safety audit

I need a simple document to record a summary of health and safety audits - this is a request for help with creating the a document rather than any of the input - It needs to give space for some narrative, provide a visual dashboard showing current overall compliance, and then breaks down by section each of the different audit areas - with space for brief summary narrative on each, a few lines which would then be a question list followed by editable tick/cross/na and a Red amber green summary for current compliance level
I've detailed more on the first page of the attached - nothing complex but I don't have capacity for it now and need a document wuite soon
