Convert PDF to Word

Hello I have a PDF file of terms and conditions that I require converting into a word document.

I need to make changes into the word document and it is imperative that the word document is laid out with each element having the correct header as per the PDF at the moment.

Needs to be in font size 11 so its easily read.

Attached is the document to be converted - return completed project required by Wednesday 14 July
