Website Designer/Developer required to redesign some pages of my website.

I am looking for a website designer/developer with years experience to help redesign the main menu and main photo upload page of my dating website

REQUIREMENT:-
1. Redesign and change the main menu layout into a drop down .See section 1 in attachment.

2. Redesign the main website picture upload page (ICON photo) with the ability to upload photo by webcam next to the manual upload photo ICON option already on this page and admin will be able to approve this uploaded picture by webcam like the manual option. This page is the 2nd page of registration . See section 2 in attachment

3. Create a graphic similar to the graphic in section three in attachment on the picture upload page to show the kind of image (photo) specification user (members) need to upload as their main website ICON photo.

I am looking for website designer /developer to help design the above requirement and fully test this project before uploading on to my site. Please note, that i have a dedicated developer that can help do the integration if you are only able to produce the design only but you must be able to show the design working on your staging server.

When providing a bid, please state if you are only going to provide design or provide design and integrate on my website.

I am only looking designers who have done similar work with portfolio and must able show previous work, do not bid if you don't have a portfolio. I need a quick turn around on this project.
