Web form to MySQL and Web Service

I require a web based form creating that will capture certain client information (around 30 fields), store it in a MySQL database, and process the information to a 3rd party web service using xml/soap.

Once the process is complete on the 3rd party the client needs to be redirected to an URL returned from the web service.

The form needs to be slick, and very user friendly. There needs to be no double-submissions so any submit button needs to disable once pressed, and there needs to a be a nice loading gif image to inform the user their details are being processed.

I have a full integration guide for the 3rd party web service that will be given to the winning bidder.

Would like to see samples of previous forms created so I can see how good they are.
