We need assistance completing a web application. Our client has requested that we build a complex online questionnaire. It will be set up individually for each user with a login and pre-populated information. The results will then need to be collated and reported in a management screen.
We have worked with a programmer already who has designed the questionnaire in Semantic UI. Unfortunately he cannot complete the project. The remaining work can be split into two phases:
1) Finishing the questionnaire by connecting it to the "initial load" data (login and pre-populated fields) and then posting results back to the database. There may be some adjustments to the questionnaire design as well which will require familiarity with Semantic UI.
2) Creating a management screen to set up new projects and output reports on ongoing/completed projects (we envisage csv files for both input and output formats).

We estimate that this will take approximately 20 hours of an experienced PHP programmer's time - split roughly 50/50 across the two phases.
