Our company requires collecting daily data from several supervisors about work projects that have been completed and hours that employees worked on various task, as well as some additional data (see attached form). Ideally the form would look very similar to the PDF version, have drop down menu or autocomplete fields for employee names and projects, be functional on tablets or smart phones, and if possible adaptive to add additional fields when necessary. An html version of the form was developed and can be used or modified, but processing of the form is lacking. It is essential to validate the form for essential fields, collect secure data, email the form data (ideally as a PDF or formatted to look as the online or PDF (attached) version of the form). It would be great to also collect the data in a central database in a format that could be imported to excel in the future.
