As part of an ongoing project, we require a web-based reporting system to allow people to report any type of incident from any worldwide location and ensure it reaches the right person.
The basis is:
One link to report an incident
The type of incident generates the correct form to fill out (Health & Safety, HR, Theft etc) and also generates the email that the form will be sent to.
When the form is completed it is sent to the relevant person (has to be user friendly to allow changes) and is also captured centrally to allow reporting.
The central database should allow the user access to specific categories, again HR, H&S etc and must be user-configurable.
The web-from should be a four step process:
1. What is the person reporting (Required)
2. Where & When (Required)
3. Details (Required) - Details will be one of 4 templates (HR, H&S etc as each dept needs to capture slightly different data)
4. Details of person reporting
The database should allow:
1. Each department to have a top-line summary from key data in each report (i.e. total value of reports etc)
2. Restricted access (some read only, others only access to one dept & some admin users for full access)
3. The ability to allow the recipient of the incident to add notes & mark as closed.
4. Incidents shown as open or closed.
5. Capture the details of the user who closes.
It has to run on absolute minimal power with preferably open-source software, there is little to no scope for any extra hardware.
