Estimated project duration: 30 hours
My previous developer let me down at the final stage of this and so I need someone finish off a web form for me. I have a very small deadline and so need this to be completed asap. So I will need the developer to start right away and give me a timeframe once you have looked at the code.
The form is online at http://practitionerselectinsurance.com/ and it is nearly functioning correctly. It is a multi stage form that serves up questions based on your profession. I have a file attached with a list of changes that need to be done. These may not make sense until you have looked at the form and the code.
All questions in the file need to be required. The html5 is set as required but I don't think that covers it fully. This will all need to work down to lower versions of IE.
Ideally fixed prices and if there are more requests/ changes then if possible it would be ideal to sort out payment when the changes are required by the client.

When the click on the 'click here there needs to be a pop up that has the following definitions in it:
- Minor Surgery – any minimally invasive procedure conducted under local or no
anaesthesia. ‘Minor Surgery’ excludes routine injections (which are not deemed to be
surgical) but includes injectable fillers or any spinal or lumbar injections.
- Major surgery – any invasive surgical procedure conducted under general, regional, spinal or
caudal anaesthesia or conscious sedation.
- Elective – any procedure which the patient has chosen to undergo, which is not deemed
medically necessary.
- Elective Cosmetic/Aesthetic – any elective procedure (as defined above) which is purely for
cosmetic or aesthetic purposes.
- Non-Surgical Consultation – any patient consultation, advice, conversation, and non-
invasive investigation. This includes any non-invasive diagnostic tests such as external scans.
This also includes routine (non-spinal) injections, and the taking of blood samples. This does
not include pre-surgical informed consent/counselling discussions, which are deemed to be
part of the relevant surgical category above.
