I've created a short youtube video of the exact specification I need.
In a nutshell...
Onload, a webpage will pull down a list of questions from a dataset at server side, display each question on screen with a yes, no button.
It won't refresh the screen between each question but it will transfer up the selected answer to each question as the user answers, seemlessly using ajax.
At the end, the dataset will be updated with the selected answer to each question.
A graph will be created and present to the user showing them how they scored depending on their answer.
