Magento - Add insurance option at cart stage

We need a facility whereby a customer can at the cart stage select to include insurance on their order. Only certain products will be insurable so an attribute will need to be created to select which can and cannot be insured.
The insurance figure quoted at the cart will be for all the valid items in the cart, items in the cart cannot be selected individually.
The insurance cost should be calculated via a rule, most likely to be a total of a percentage of the valid products prices.
The order needs to show that insurance has been selected in the admin, perhaps it should add it as an order line?
If a customer selects the insurance a separate email needs to be sent on order confirmation that contains the insurance details.
A report is required that can display the total of insurance sold for a given period.
A guide from the insurers is attached which gives an indication as to how they see implantation, however we are looking to fulfil the requirements above whilst being as simple as possible.
Ideally this work will be packaged as a module so that it can first be developed in a test install (which will be provided) and then rolled out to the live install.
