Ruby on Rails Input Form Reworking

I have a Ruby on Rails app which allows users to upload their own items and add information about people and locations.
The information relates to life events, such as births, marriages and deaths, each of which can have a location associated.
So an Item has zero or many People which have zero or many life events which have zero or one location.
The model works fine, but the entry forms are a bit clunky and need to be reworked.
For example an item may contain the following information
John Smith
- born 23/1/1880, London, England
- died 16/5/1935, Cambridge, England
Jane Smith
- born 25/2/1882, Cambridge, England
- died 1/8/1942, Norwich, England
Current workflow:
1 - Locations Page
- Enter Locations (London, Cambridge and Norwich).
- These locations are made available from the People page
2 - People page
- Enter John Smith
- - Add 'Birth' fact
- - - Add date 23/1/1880
- - - Select Location London from drop down
- - Add 'Death' fact
- - - Add Date (16/5/1935)
- - - Select Location Cambridge from drop down
- Jane Smith
- - Add 'Birth' fact
- - - Add date 25/2/1882
- - - Select Location Cambridge from drop down
- - Add 'Death' fact
- - - Add Date 1/8/1942)
- - - Select Location Norwich from drop down
What I'm looking for is to be able to enter the location at the time of creating a fact so the separate location creation page is removed entirely:
Required Workflow
1 - People page
- Enter John Smith
- - Add 'Birth' fact
- - - Add date 23/1/1880
- - - Enter Location London
- - Add 'Death' fact
- - - Add Date (16/5/1935)
- - - Enter Location Cambridge
- Jane Smith
- - Add 'Birth' fact
- - - Add date 25/2/1882
- - - Select existing Location Cambridge
- - Add 'Death' fact
- - - Add Date 1/8/1942)
- - - Enter Location Norwich
So the location form would allow a user to enter a new location or select a location already entered. This may not necessarily be a drop down but may be a popup window that shows a new location form and radio boxes for locations that have already been added. Something like that.
Additionally I'd like to have all locations from the database available to prefill the forms - I believe there is a plugin for this, and the ability to get this in within budget would be a bonus, though not essential.
Please note, due to behaviour of some PPHers, I do not and will not negotiate on price after the bid has been submitted. Please only enter a bid if you think you can complete this for the bid submitted.
