Please read all of the spec including the attachment prior to submitting a proposal.
I need an OnLine form creating that will allow me to send a URL to a customer that contains their customerID and an additional parameter labeled StarRating within the URL.
It should incorporate full SQLInjection prevention.
The page should display well on both desktop and tablet. not necessary to reformat for a smart phone screen. It can incorporate scroll bars.
It should work with all up-to-date browsers. (Mac and Windows) (Safari, FireFox, IE, Chrome)
I want this writing in vb.net or c#.net and want the code so that I can make any tweaks I need as I develop in both languages.
The form will comprise of text boxes and checkboxes. There will be no formatting of text required in the text boxes. Plain text only.
There will also be the ability to upload an image which the end user can upload and save. This can be limited to a 5mb jpg/png file.
The user should be able to click save at any time and exit the form and resume it at a later time. This is one reason for the customerID within the URL (as a querystring)
I need the form creating and the code that loads and saves the data into a table (mysql) that can reside on the same server.
If the customerID doesn’t already exist in the table then a new row will be created for that customer ID and futures edits will amend that row.
The online form needs to include our AVN Logo, a static title and some additional text to explain to the user what is required.
The form should be split in to two different sections.
If the StarRating parameter (wihtin the querystring) is set to 0 then Section 1 will be presented and Section 2 will only become visible once section 1 is complete.
If the StarRating parameter is >0 then All of the checkboxes within Section 1 will automatically be set to TRUE and Section 2 will be made visible.
I don’t mind how these sections are differentiated visually. but it will be better if the sections can collapse rather than the user having to scroll a lot from top to bottom.
I want the form to be able to scroll but always show a save option in view. This can be a header or a footer.
If there is an online solution that does this rather than it being a coding exercise, that’s great, I’m still happy to pay for this to be created using that technology but it must allow for the save and continue later option and allow me to extract the data as and when I need to and the data will be linked to the original customerID that’s passed as part of the URL. Mail chimp doesn’t seem to work in this way.
I have attached the remainder of the spec as this box won't accommodate it.
