Create Online Order Form for website - used by sales agents to place customer orders

Three databases required:-
1) A Customer File
2) A Product File
3) A Sales Agent / User File - 2 authority levels, Sales Agents and Managers

Once an order has been made, an email will be sent to two emails addresses (so that if one fails the other will still be processed) and also to the customer confirming the order. The order would then be visible on enquiry screens within the website.
I need each sales agents to be able to enter a new customer and order. Each customer needs to have an account number so that their name, invoice address, delivery address etc details are recorded. I need to be able to print invoices and export data to excel.
Customers once setup need to be able to place an order themselves. The order must be linked to a sales agent as a set commission is paid on every sale to the sales agent of the customer.
When a sales agent enters an order this needs to be a simple input page, each product will be listed in a row of a table, all together 11 products plus a number of variances. Therefore a table with approximately 20 rows and a simple 'qty' entry field per row/product.
Debit/Credit card processing required. I will open a merchant account and need the gateway setting up - standard template can be acquired from merchant provider.
