ASP.Net HttpWebRequest, pass an object to web service using application/x-www-form-urlencoded

we need our existing vb.net example code fixed (see attached) and it needs to work in Visual Studio 2005 and .Net Framework 2.0
on clicking the test app button, the code needs to call the simple web service and pass the simple object to the web service using content type "application/x-www-form-urlencoded" and receive a success status back and for the web service to successfully receive the object.
curently the code returns an error of "The underlying connection was closed: An unexpected error occurred on a receive."
we know we can simply add a web reference to the web service and call it that way but that's not what we need. we need it to be coded using HttpWebRequest and content type "application/x-www-form-urlencoded"
we just need the source code changes sent to us so we can change our source code and successfully run the software.
