We require an affiliate agreement

We are a price comparison website for money transfer companies. After a year of listing companies and offering conversions and leads for free we now want to set up a commission based business model depending on performance. Therefore we require an affiliate agreement (terms and conditions included) that we can send to all companies listed on our site. The agreement should have an introduction to our services and an affiliate request form.
