Document Editing and Redesign

I need two blank versions/copies of this document with handwriting removed.

Requirents:

1. Document needs to be in black and white
2. Background needs to have the hologram underneath as well as the subtle texture design as an official document. Logo in the middle of the document will be provided.
3. All handwriting needs to be removed.
4. Top left and top right corner, serial number of the document needs to be changed (x2)
5. Bottom left corner, stamp needs to be removed just leave the square and “Official Stamp”
