URGENT - Desing a logo

I am urgently looking for an experienced designer to design a logo base on the attached image. I will provide a specific image to go in the middle and you will add a background. I will give the copy.

You will provide all the sources file ai, jpg, psd, font, etc.

You need to be able to start right away.

You will be prepared unlimited modifications if required.

I will give more details once your proposal have been accepted.

You need to have a portfolio, have already provided service on People per Hour 100% positive feedback.
