Wordpress - SMDSY New online form and report

We are looking to add a new form to our existing website to allow staff to log forklift vehicle checks on Site Connect. https://site-connect.co.uk/

Site Connect is a company platform that allows staff to complete online forms to send information to the office.
The platform is built in WordPress and already has a number of sections already. This is an active site and has been running for a number of years.
The development of the new form should match our existing styles and follow similar rules for type fonts and buttons used. Assets can/should be reused from other forms on the platform.

We the form to be created as shown on the PDF supplied.

• The submission form can be made by any ‘Smd_stockyards’ User.
• The Dashboard can be seen by ‘Administrator’
• The email notification should be sent to the same email as the other existing SMD Stockyard location.

Throughout the pdf are RED TEXT pointers and comments, these should help when understanding which cells are used in each step.
