Wordpress/elementor Custom form and database feed

My name is Ivan. I got an old website, that I am currently refurbishing for my new business idea. I need a new WordPress/Elementor (or coded if that's easier for you) form for my site.

1) I need a form created. Form is a questionnaire for tutors/teachers. It has two questions with simple dependencies where next question depends on previous answers.
2) Each form submitted by applicant a) should send an email confirmation to applicant's email and to me, b) each submitted form should feed into excel/table database accessible from admin side/panel.

Is it something you could help with?

Once this stage is complete, there will be further steps, like: advancing database to be generate emails to selected applicants etc.

Please let me know if you could build custom form and set database feed and admin access.
