UPDATED: 2 Web forms with database connection

Description: We now need two web forms (on two different web pages) that candidates/applicants will complete. The forms must populate an existing internal database (dbase IV) for use with Goldmine 6.7 CRM software. Good UI design is a must (as you'd expect) and must be consistent with the rest of our website. We currently use a 3rd party form on our site that sends form data by email (which is no longer ideal). We would need some validation on the form to ensure the data is consistent with our tables and there are 1 or 2 calculated values that need to go into the database.
