Subscription page of website updated / redesigned

We currently use Bold Subscriptions on our website and have existing subscribers that we would like to keep as they are. We would like to keep their account settings the same, so they can log on to their account still and have same functions that are in place now. We will consider migrating to another subscription app i.e ReCharge or similar if we have the same functions.

Moving forward we want to change our subscription format. There will be a three question flow before user chooses a subscription box. I have attached a couple of sketches to illustrate this. Please advise can this also be done on Bold Subscription or is it better on ReCharge? Can it be coded in to website?
