Straight Forward Form Embed Code Update

We are trying to modify the code of an existing form that we are looking to embed on our website. I have already attempted some modifications, however I appear to have missed a closing tag which means it is now interacting with elements outside of the form.

The modifications we require include:
1. Comparing both pieces of code (original versus modified) to find the problem tag
2. Adding required padding or spacing where elements are too close
3. Adding captcha to the form if possible (groundwork is already there in the code)
4. Prevent the form from loading a new tab upon submission, load confirmation in same tab

These should be quite straight forward fixes as the code is already there. Happy to share more information as required.
