SEO help for my site with url and keywords

Currently I'm not ranking for my website on a not too difficult keyword. I need to know...

For each individual page (7 pages) I require

1.) What to name page title for optimal SEO
2.) What page URL to use for optimal SEO
3.) What data to input in the meta data box data for optimal SEO
4.) What data to input in the keyword data box data for optimal SEO
5.) What keywords I should use for each individual page amongst the pages. I’m concerned about repeating myself too much and being penalised.

My business (childrens entertainer) operates throughout a few major cities so trying to target them (and possibly some of the smaller in between towns).

I have a few rivals who seem to be ranking for all the terms I want. I would expect you to research them too to see how I can better them (sorry if this sounds obvious).

I design my site in Adobe Muse to please bear that in mind (I have no html knowledge except inserting html code from the insert html option as in the use of Google analytics code), however I can amend all above stated requirement.

Successful applicants would ideally have clients they can prove they have ranked on page 1 Google for. The fee will include on going support for a few days in case I encounter any difficulties.
