ONLINE CONTACT FORM WITH PDF DATA FIELDS TO UPDATE ON WEBSITE

currently we have a referral fee contact form that has to fill out and completed on our site for us to pay out a referral fee; the agent that shows a property for rent must also download and fill out two other forms ( the referral fee pdf document that must be signed by the agent and his/her broker and also provide us with a w9 from their brokerage that they must obtain and email over); we are wanting to streamline the process

would like the ability for an agent to fill out the realtor contact form online, and is then able to fill out the pdf form ( referral form) ; the referral form must be signed by their broker and the agent also; so not sure if there is a more streamlined route to process referral fees instead of having to have someone fill out the online form, then print out the referral fee pdf, sign it and then email back to us in addition with the w9 pdf:

we would like to have the agent fill out the online form, the referral form which would then be electronically signed by the agent and his/her broker, upload a pdf w9 and then once complete would be emailed to us for our agent to sign and our brokerage on the referral fee form for a complete transaction

we would want the project to be hosted if needed on a page we select instead of it being hosted by an unknown provider; we are fine using host gator or godaddy

we have had two separate developers unfortunately not complete the prior project after paying both 350 each; the first developer provided a sample link that was almost complete then never heard from him after he was paid and the second developer wanted payment well before the form and functionality was complete; so have paid both with zero results

attached is the referral fee agreement that the agent will have to complete lines 1-6, lines 13-20, and lines 63-64 were the agent and the agents broker will have to sign; after that form is completed; then the completed referral fee and the brokerage w9 that they provide to agents ( the attached example will not have to be filled out yet we will need to receive their w9 that they provide already completed to their agents; so the agent will need to be able to upload or email the form back to us).

we use docusign for many transactions so the project should include some form ( ie docusign ) or someway for the agent, the agents broker, then our agent, then are broker to sign in chronological order

looking for someone that can actually complete the entire project and take pride in workmanship instead of just getting the money out of escrow and not completing the job
