Mobile friendly web app HTML5 / Javascript / Bootstrap

I am looking for some development for a mobile friendly web application. The design for some initial pages are attached, the server side is already written and a demo web site can be see at

and this site shows the server communications already working, the script is provided

page 1)
shows a map with live tracking of icons (from server and already done), click on target icon fly's map to device location

user can add or remove previously added addresses

page2)
on focus on input, it shows map and current location, on scrolling map current location shows address (from server and already done). click on target icon fly's map to device location

page 3)
on typing text, as user enters the text is searched as address (from server and already done) and the user can select an address. goes back to page 1

must work on android and iPhone, pure HTML5 / Javascript (as the script is now)

looking for this to complete within 14 days of starting. windows remote desktop provided to upload scripts and test your code
