I need someone to complete my website which is about 95% done. Details below.

Here is what I need done to complete my website which is about 95% done.

1. Change, or enlarge, the black header area which is now 2 ¼”…enlarge to 2 ¾”.
2. Create a .SWF video (or maybe it is called a Flash video) which will be inside of the header and will have the following things in it which will appear at the top of EVERY PAGE.:
a) On top…. Name of the non-profit organization which is “Lillie S. Cross Foundation, Inc.”
b) Middle space – Photo of Lillie S. Cross (centered under the above title)
c) Bottom row/space – Insert this phrase, “To Whom Much is Given…. Much is Expected in Return.” (centered under the above photo)

I would like a quote of how many hours it will take to complete the above tasks… and the final cost for me.

3. I MAY need to have an application blank attached to the “Application” page. I need one where the user can type on it and the click “submit” after completion and have it come directly to me and two other people who are volunteers. (What would this added cost be if I cannot do it myself?)

Since my website is about 95% done, as I said, it is NOT published on the Internet yet. Based on the 2 files (Pages) attached, you can see what I am referencing relative to where the Flash file needs to be placed. I am attaching the Home page and the "Application" page.
