I need a single page online interactive questionnaire developing

General information for the website: WHO 5 Well-Being Index
Num. of web pages/modules: 1
Description of every page/module: This will be a single php page containing an interactive questionnaire. The questionnaire will probably need developing in HTML5 and JavaScript to handle the animated 'result meter' that will be displayed when the questionnaire has been completed.
Description of requirements/features: We want it to work on the raw score rather than percentages and we imagine that the person fills in the boxes, clicks submit and then the meter reading shows their score with the needle going from 0 to the top score and then settling on their actual score.
