I need a simple web application creating

General information for the website: Attractive 5 Question Web App
Description of requirements/features: I have started a small web (audit questionnaire) app here: http://www.futurelearningsystems.co.uk/how-lean-am-i/

I don't have time to finish it and need some help with the design. The app does the following:

1. Shows an intro page
2. Asks the user a question and shows a spider diagram based on their response 5 times
4. Shows an overall results page

It's as simple as that really and most of the functionality has already been written. You may want to start again or continue with what I have done, my main concern is an attractive responsive design. The spider diagram uses Chart.js which is free.

I can provide the full set of 5 questions to you, the question models are all exactly the same just with different text.

Ongoing work for someone who can do a good job at a low price.
