HTML Form with Calculation

Hello

I have a Excel calculation that I want turning into a form that I can embed on my WordPress website.

The calculation is here:

Once the form has been submitted I need to send an email with the responses to Admin and redirect to a Thank you page.

Form needs to be mobile responsive.
