Doc 2 Form

I have a document that I need transformed into web format. More specifically as a form. The form will populate a Google sheets spreadsheet through our API, so I just need you to handle the frontend.

Please note that data validation is a must. Also anything with a '___' is an input field. Anything with a bullet point is a checkbox.

Moreover, if a See Comments checklist is 'checked' then the associated comment section needs to be validated with an input.
