Custom website connected to web services

I have a SQL database created along with a web service that interacts with this database. I need three web pages built out to call these methods which will display and allow updates to this data. Attached is a more detailed guide to explain the database and service installation process (MS SQL Server and IIS needed), along with the page design.

The three pages will include a home page which is simply a display of data. It will use one method called three separate times with a different parameter for each of the three business units. You will need to be able to get a count of items returned from the method call for display (list.count). The home page will link to either a detail page for the business unit or a search results page that shows details for a single item (policy).

The detail page lists all open items under a business unit. A link should allow a pop-over window to open where the user can enter comments and their user name. Submitting the form calls two methods - one to add the comments and one to close the item.

The search page displays all items (open or closed) for a single item or policy. It will also display the comment if it is present for a closed item. Users can click a link to dd comments for nay that are open.

A rough design concept is included in the documents, but I am open to minor changes in design.

Important - I would like to have this done within a week if possible. Please include your availability to start on this immediately in the response. Please also include a price to create these pages. If changes are needed due to requirement changes I am willing to add more to the amount paid - I will be fair for any extra unexpected time needed to complete this.
