Create an simple and user friendly lead magnet questionnaire

I have a questionnaire produced in Excel that I want to turn into a lead magnet that people can download from a webpage and get responses to their answers. I want to turn the spreadsheet into a format that is more visually appealing and easy to use.

They also need to be able to complete it with their partner to get comparison results and inspire conversations around their similar and different views on money.

They will need to input their name and email address to be able to access the results.
