General information for the website: PDF to HTML
Description of every page/module: I need to create an html from 2 page pdf form.

The html should be compatible with contact form 7 in wordpress. So little bit knowledge of wordpress is required here but not required as I can explain how to get it done.

It quite easy who knows html and mostly can use tr td tags of table tag in html.

Please get in touch so I can explain better and can send pdf to check the form. There is no fancy style or anything like that.
