Contact Page and Thank You Page for website

Specification for Contact Page for HitTheGroundSprinting.com
I would like a contact page for my website the fields for which are as follows:
Forename
Surname
Email
Subject
Comments
All, except the Surname, are to be compulsory (and have an * indicating this)
The fields are to have up to 75 characters allowed in each except the Comments field which is to have 500 characters
There are to be two buttons, a ‘Send It!’ button to send the form and a ‘Clear Details’ button which will clear all the details from the form to start again.
If, when the ‘Send It!’ button is pressed, any of the compulsory fields are not filled in the edges of the compulsory boxes not completed are to colour red and a message appear saying ‘Please complete the Compulsory Fields!’. This can be either a message box OR appear on the form itself. I DO NOT want the message to be on a different form.
When the form is complete and the ‘Send It!’ button is pressed I wish a ‘Thank You’ form to appear with text thanking them for sending the details.
This form is to be sent to my business email account, and there must be NO messages stating an email is being sent.
