Contact form - simple contact form activated

Website - I am having issues with the contact form.

This will not take more than an hour.
I need a simple... Contact form - simple contact form activated and set up to work...

1: I would like someone to configure my contact form .
2: It is sending blank information to me. I tried to amend it but now there is just an error message where the form should be.
3: ALSO.....After that issues have been sorted. ..I would like a box to be placed on the main page higher up that says contact.

This is a simple job for someone who understands Wordpress.
