Automated Proposal System Integration Into Existing Website

This is the website.

We want to change the quote page into the screenshots(1.jpg and 2.jpg) attached and we also want to be able to make all sections except F and A editable through admin panel to add more services and change their pricing if need be.

Once the client has filled out this form, they will get the details of all their selected options and the total into their email. A person from our company will also be cc'ed into this email.

Next, the person from our company will also a document(LOE) into their email as an pdf with specific fields updated according to the services they've filled out in the form above.

Our website is set to go live next week and we would like this done at the earliest.
