Application Form

I need Someone build this application form in Wordpress , it does already exist( although they say they have deleted it) and i have paid for it, as i have just copied a mock up from my developers website. they seeming what to charge me again for it , I would sooner pay someone else to do this, I obviously need a blank for to be able to unload files,
