Add New Test Questions to existing Laravel membership system

Basically this is adding a simple multiple-choice quetion test for our members to do. If they pass it this shows on their membership, if they fail it does not (and they can try again).

We will have 5 questions and the pass mark is 4, however both of these values should be defined in the database so that we can have more, or fewer questions and we can change the pass mark.
