Develop a Moodle YUI module for text input as a modal pop-up

Description of requirements/functionality: We are developing a local plugin for our Clients Moodle LMS. The plugin will be an online form which displays dynamic information taken from the moodle database and will also allow a user to add comments to specific areas of the form. We want the user to have the ability to add text to the form via a modal pop-up box. When a link is clicked on the form the pop-up will be displayed. The user will enter the text, click a submit button which will then save that text to the database and refresh the main online form with the newly added text.
Specific technologies required: JavaScript, YUI, PHP
Extra notes: The version of the Moodle LMS is 2.7 and we would like the modal form to be developed in accordance with Moodle guidelines found here: https://docs.moodle.org/dev/YUI/Modules.

The plugin we are developing is an ad-hoc plugin, designed specifically for our client and is not intended for use by others. However, if possible, the YUI pop-up module will be designed so it can be incorporated into other plugin forms.

We have put an estimated job duration as 1 day or less as a placeholder. We are happy to discuss further with the successful candidate to get a better feel for time and budget needed to complete the work.
