Description of requirements/features: I need a web form recreating with validation. There are nine fields - most are standard email, phone etc. One allows 5 codes as valid entries. Appropriate error messages are required. The form should send to email and redirect to a thank you page.
The form exists on an old page but is not working when copied to a new site and therefore needs tweaking / re-doing
