VTiger modification with PHP form

Hi.
I need someone who can work with me a few hours a day modifying the form I have made for VTiger.

I need to work with direct programmer. No TEAMS, MANAGERS etc please.

It's separate form, purely build: http://bios.fepfs.co.uk/form/

which create the final record in the VTiger custom module.

I need some modifications, to be done in the next 2 (max 3 days). This would be per hour job, as I would make updates on the go. I would expect I need initially 10-15 hours of work.

I can start tonight / tomorrow.

I have a current list of things to do, some are really small, some bigger, like creating a pdf report (inserting values in prepared word template)

I need someone who knows Vtiger CRM, as this works in this environment.
