utility to draw a line on a pdf document

I currently use the PDFtk library (via a shell command) to populate a PDF file with data in order to create a populated PDF form
However, on some pages, I need to be able to draw a line through some of the sections to show that they have been left intentionally blank
For example, on page 5, if I don't want to put emergency contact details, rather than just leaving it blank, I want to draw a line through that part of page 5
In an ideal world I would have a small utility that I could call and "attach" a text file defining the lines I want drawn.
The text file would have the format: page number, startX,startY,endX,endY (measurement I guess would be in inches, but I'm flexible)
So for example: 5,1,1,2,2 .... would draw a line on page 5, starting at 1in in from the left, 1in from the top and finish as 2 inches from the left and 2 inches from the top
I would like to call this utility either by a function, or a shell command and in my mind it would have the following format:
program_name
eg if we assume the executable is called "pdfdraw" and the list of drawing instructions is called "instructions.txt" then the following:
pdfdraw c:\documents\sample1.pdf c:\documents\sample2.pdf c:\documents\instructions.txt
would then open sample1.pdf, draw the lines as defined and output a file called sample2.pdf
I will only ever want a thick black line
