Single Web page PHP interface with SQL based on User defined input with data return

Looking for a single page "Dealer locator" based on a User Defined input. Will require SQL & PHP. Process has been designed in Access however needs to be translated into SQL that can be accessed via our website.

The only changes to the DB should be done by us never the customer, they should be able to look up their Rep by entering in their Zip.

The Table should only need to be updated on occasion looking at 1-2 times per year at the most, but should only need to update the Rep Table.
