Simple PHP Web Application Completion

Kind of development: Customization of existing website
Description of every page/module: I have a simple questionnaire application. It has been ~70% finished.

It is database driven with a dynamic frontend in JQuery.

It currently has 1 category of questions, but needs to be changed to 3 level arrangement.

It needs to accurately collect the data - I dont believe all answers are going in the database.

It needs to have the user model changed so many users from one organisation can complete the exercise.

General tidy up and hardening of the application.

Deployment onto my server.

I need a fixed price project and we will jointly agree a detailed spec.

It needs to be finished in the next week or so.

We can discuss scope in more detail if the above sounds fine.
