Send form data to Google Sheets

Hi - I'm looking for data that's entered in forms on our website to be sent straight to Google Sheets.

An example of the form can be seen here on the right hand side - https://touringhighlights.com/tour/north-thailand/ (name, email, phone). This form is also on every page (& future page) which is /tour/ in the URL

We're looking for this data to be sent automatically to a Google Sheets file with the following data;
Tour (e.g. the link above would be north-thailand), name, email, phone, agree with terms & privacy tickbox.

We would like a price to also get another column in Google sheets which tracks the source if possible (e.g. if they came from Facebook, email, Google, Bing, etc?).

At the moment, when a form is submitted we get an email only - this data isn't stored anywhere.
