PHP slider form

Hi,
I have been building a slider quote form in html/php I have got it to where I need it to be in terms of fields ect but I have maxed out my php understanding now and I'm unable to get it to send an email with the completed form results. I'd also like the form results to be able to redirect to a url of my choice.

I just need one of you php experts to get it working for me and easy to edit for future requirements.
I'm happy to send original files.
