PHP MySql Website, Application

We are an Asbestos Survey company who carry out Type 1 and Type 2 surveys for a wide range of customers. At present we create our reports and email these to our customers these reports are generally in .pdf format.

We would like a new website developing in PHP and MySql, our website is for our customers to visit to view, search and print there documents to save us from emailing them so basically a document management website.

We need to be able to add and remove companies (our customers), these companies are only allowed to see there data and not any other companies. Each company will have many representatives who will be allowed to logon to the website to view these reports and only sites that they manage, a representative will be will either be allowed to view one site or many sites within that one company.

We need to be able to upload documents all formats to the following sectors.

Surveys
Remediation
Training
Other

We may need to add some more to these later so I would like them to be dynamically loaded from MySql. Within these sectors we need to be able to add, view, edit, delete, search.

We can discuss the table fields once the price has been agreed, these tables will consist of no more than 10 fields.

On the admin end of the site we must be able to add new companies, representatives and set the appropriate permission levels to these representatives based on which site they look after, they can add, remove and delete documents and run reports, we must also have an email notify to inform the representative that a new document has been added, removed or deleted from the sites that they manage.

The website must have full error checking and validation.

I would like the site and navigation to look similar to the attached example (Example 1.png)

I need this website finishing in 5 days with a working example to view and test by Monday 12pm. (midday) and also test along the way to see what other information may be needed if that’s the case.

I would preferably like a English spoken person which I can have skype calls with.
