PHP data tables, with some configuration

I would like to adjust a few pages as per images. No CSS / colours are needed. Just raw page.

main page, see image attached, I have drafted that table + filters/query fields in order to query the table. I can share the code of this page. Fields are just `invented`, same for filters fields, need to be adjusted to give full freedom to the user to filter the table.
