PHP contact form with reCAPTCHA

Description of requirements/features: Re-posting this job since it wasn't delivered correctly the last time:

PHP contact form with reCAPTCHA
- Simple website contact form with standard fields to collect (name, email address, message) and the option to easily add more fields
- Form should send email to specified address when submitted
- Needs to be re-usable across multiple sites, so easy to set up email addresses and such
- Needs to use Bootstrap 3 standard form markup
- Fields need to be easily set to mandatory/not mandatory
- Needs server side validation as well as JavaScript validation (validation should check for mandatory fields/minimum characters and properly formed email addresses)
- Validation should populate Bootstrap standard error messages and form field highlighting (both server-side and client-side)
- Needs optional setting for enabling or disabling reCAPTCHA
- Submitting the form should be AJAX where possible, clearing the form and presenting a success/error message close to the submit button. This should use standard Bootstrap markup.
- The form should function correctly with JavaScript disabled (I'm aware the inline validation won't work, but would like the PHP to reproduce it as well as possible)
- reCAPTCHA should also work with JS disabled; there is a non JS version and although it's a little clunky, I'd like to be able to utilise it
- Ability to set the SMTP server for sending the mail
- Well documented code
- Should, where possible use third party open source plugins (for example for email address validation, jquery validation etc.), no point in reinventing the wheel!

Requirements are negotiable!
HTML/CSS can be provided if necessary, for mocks of various states.
