PHP Application Development

We are seeking a confident and industrious freelance PHP developer for a number of different ongoing projects.

We have an initial PHP project, if successful this would turn into numerous future projects which we currently have on going.

This first project is to develop a Customer Satisfaction Survey Application. The application UI has already been developed in HTML but requires PHP development to make the application functional. A HTML Demo of the application can be provided if necessary.

The application requires a user login script, CRUD functionality and email via templates. The application will require 3 access levels: User, Supervisor and Administrator. The general use is the user logs into the application and completes an online form, the results of this form are then stored in a database and a copy is emailed to the customer, the results are then displayed within the application.
